A record should say exactly what it can support.
The fields, states, and limits used to keep important claims inspectable.
Identify the subject and method.
A useful record names what was examined, the version or state available, the method used, and the time at which the observation was made.
Retain both success and failure.
Corrections do not erase the failing state. Refusals, missing inputs, and unrun checks remain part of the result.
Use precise status language.
PASS, FAIL, UNVERIFIED, NOT RUN, BLOCKED, and superseded carry different meanings. A green local check does not establish a hosted, production, device, legal, or customer claim.
